If you have been following Mushoku Tensei: Jobless Reincarnation and noticed Shizuka Nanahoshi’s sudden, alarming health decline, marked by worsening coughs, fatigue, and coughing up blood, you might be wondering what is actually wrong with her. Unlike the native inhabitants of the Six-Faced World, Nanahoshi is an ordinary human from Earth who was pulled across dimensions.
That single origin point is the root cause of a terrifying and mysterious condition known as Drain Syndrome (often referred to as mana sickness). Here is everything you need to know about why she is sick, why standard magic makes it worse, and how it impacts the story.
What is Drain Syndrome?

Drain Syndrome is a rare and fatal condition that targets individuals with little to no internal mana capacity who live in environments heavily saturated with magic.
- The Earth Biology Problem: People born in the Six-Faced World have spent millennia developing a natural immunity and biological capability to process ambient mana. Nanahoshi lacks this entirely because she is from Earth.
- Passive Accumulation: Even though she cannot generate, cast, or store mana internally, her body constantly absorbs ambient magical energy from the air, food, and surroundings.
- The Toxic Buildup: Without a natural biological mechanism to filter or release this energy, the ambient mana builds up inside her over the years until her system begins violently rejecting it.
Symptoms and Why Healing Magic Fails
As the accumulated mana reaches toxic levels, victims experience severe physical symptoms that mimic a lethal illness.
- The Progression: It starts with a persistent, dry cough that progressively worsens, eventually leading to coughing fits that produce blood, severe exhaustion, dark circles under the eyes, sudden collapse, and unconsciousness.
- Why Magic Makes It Worse: In a world built on magic, a natural instinct is to use Healing or Detoxification Magic. However, standard healing spells fail because Nanahoshi’s body reacts violently to external magic inputs, making standard medical interventions potentially fatal.
Is There a Cure for Nanahoshi’s Illness?
Drain Syndrome has no permanent cure because Nanahoshi cannot stop absorbing the ambient mana required to live in the world. However, her condition can be managed temporarily. Drinking a specialized tea brewed from Sokasu Grass safely flushes the excess accumulated mana out of her system through waste, pulling her back from the brink of death.
Managing this chronic, terrifying condition forces Rudeus and his allies into a desperate race to find a long-term solution so she can finally find a way back home. However, in Episode 8, which was the start of Chaos Breaker in Mushoku Tensei, we saw Nanahoshi coughing blood, but in the manga, we are way ahead.
